What is betaine HCl?
Your stomach normally makes a strong acid (hydrochloric acid). This stomach acid breaks down food and activates pepsin, the enzyme that breaks down protein.
Betaine HCl is hydrochloric acid in pill form. Once it reaches your stomach, the acid is released and makes your stomach acidic again. That's why it's sometimes called "artificial stomach acid."
In the US, it's easy to find in the digestive enzyme section, and many products also contain pepsin.
What is low stomach acid? Who it's for
Low stomach acid (hypochlorhydria) means your stomach makes too little acid. It becomes more common with age, with long-term gastritis, and with long-term use of stomach medications.
Without enough acid, meat doesn't break down well, so you feel bloated and gassy. Surprisingly, heartburn or acid coming up can sometimes be caused by low stomach acid.
If several of these apply to you, low stomach acid may be worth considering.
- □ You're in your 40s or 50s or older
- □ Meat and protein make you feel heavy and stuck, but plant-based meals are fine
- □ You often burp, get gas or feel bloated after meals
- □ When meat leaves you feeling heavy, vinegar or apple cider vinegar settles your stomach
- □ Your nails break easily, and you have anemia or eyelid twitching (signs that can show up when nutrient absorption drops)
- □ You've been diagnosed with long-term gastritis or atrophic gastritis (a thinned stomach lining)
Who should not take it
Betaine HCl is literally an acid. If you already have enough stomach acid, it irritates your stomach lining.
- People with stomach ulcers or severe gastritis or reflux: heartburn may get worse. Avoid it.
- People with healthy digestion: you don't need it, and it may cause heartburn.
- People taking acid-reducing medication: the two work in opposite directions. Talk to your doctor first.
How to take betaine HCl and how much
Bottom line: right before a meal, starting small.
- Start with 1 capsule. Take it right before a meal that contains protein, or with your first bite.
- Watch for a few days. If your stomach feels more comfortable and the bloating eases, it's working for you.
- Increase slowly if needed. Some people go up to about 500 mg per dose, 3 times a day (with each meal).
- If you get heartburn, go back down one step. The amount just below that is your right dose.
Taken on an empty stomach without food, it irritates the stomach lining. Always take it with a meal.
Many say it's fine to take long term if you have chronically weak digestion. Either way, the rule is simple: does your stomach feel comfortable after taking it?
What to pair it with: pepsin and digestive enzymes
| Combo | What it does | Best for |
|---|---|---|
| Betaine HCl + pepsin | Restores stomach acid, and pepsin breaks down meat protein | People who feel stuck after eating meat (the basic combo) |
| + pancreatic enzymes | Further breaks down carbs, fat and protein in the intestines | People with heavy bloating and gas after meals in general |
Pepsin only works when there's acid. That's why most products put betaine HCl and pepsin in one capsule. If your digestion is weak overall, adding a multi-enzyme blend is another option.
What to check when choosing a product
- Whether it includes pepsin — labeled something like "Betaine HCl & Pepsin."
- The amount per capsule — since you start low and work up, a capsule that isn't too strong is easier to adjust.
- Stomach-protecting ingredients — if you're worried about heartburn, some products include ingredients that protect the stomach lining.
Medications and conditions to watch
- Caution Stomach ulcers or severe gastritis — Irritates the stomach lining and may worsen heartburn — don't take it
- Check Acid-reducing medication — They work in opposite directions — talk to your doctor before taking it
Combinations
Works well with: Digestive enzymes
Frequently asked questions
Should I take betaine HCl before or after meals?
Take it right before a meal or with your first bite. It needs food present to help digestion. Taken on an empty stomach without food, it may cause heartburn.
What if betaine HCl gives me heartburn?
That may be a sign you already have enough stomach acid. Lower the amount one step, and if it still burns, stop. If heartburn continues, get your stomach checked by a doctor.
I have acid reflux. Can I take a stomach acid supplement?
Sometimes reflux symptoms are caused by low stomach acid. But if your gastritis or reflux is severe or you have an ulcer, you should avoid it. If you have a diagnosed condition, talk to your doctor first.
How can I tell if I have low stomach acid?
If meat makes you feel heavy and vinegar settles your stomach, it's worth considering. A simple check is to try about half a tablespoon of vinegar in water when you feel heavy after a meal.
Can I take betaine HCl long term?
Many say it's fine long term for people with chronically weak digestion. Judge whether it suits you by whether your stomach feels comfortable after taking it. If you get heartburn, lower the dose or stop.
Is betaine HCl the same as TMG (betaine)?
Both contain "betaine," but they're used differently. Betaine HCl is for replacing stomach acid, while TMG has no acid and is used for other purposes. If digestion is your goal, choose betaine HCl, the form with the acid attached.
