Folate: who needs it, when to start, and how much

Folate is a B vitamin that's essential for women planning a pregnancy. It's also used for blood vessel health and processing female hormones. 400 mcg a day is the baseline, and no more than 1,000 mcg. Regular folic acid is enough, but if you have the choice, the active form (MTHF) is a safe pick.

Who it’s for

Women planning a pregnancy, women in their mid-to-late 30s and older, people looking after blood vessel health

Daily dose

400 mcg a day as a baseline, no more than 1,000 mcg (including what's in your multivitamin)

When

If you're planning a pregnancy, take it every day from 1–3 months before conception through week 12

Best form

Regular folic acid is enough; if you can choose, the active form (5-MTHF)

What is folate?

Folate is one of the B vitamins (vitamin B9). It's plentiful in leafy greens like spinach.

It does three main jobs in your body.

  • Making cells: It's needed to make red blood cells and new cells. That's why it matters so much in early pregnancy, when a baby is growing fast.
  • Lowering homocysteine: It turns homocysteine (a substance in the body that can damage blood vessels) into harmless compounds. It works as a team with vitamins B6 and B12.
  • Supplying methyl groups: It hands out methyl groups (small parts the body attaches and removes in chemical reactions), which are used to process female hormones and make neurotransmitters (brain chemicals involved in mood and sleep).

Who needs folate

  • Women planning a pregnancy: This is the clearest reason. A baby's neural tube (which becomes the brain and spinal cord) forms in early pregnancy, and folate is essential at that time.
  • Women in their mid-to-late 30s and older: It's needed for processing female hormones, so it's often recommended as part of a basic routine.
  • People looking after blood vessel health: Along with vitamins B6 and B12, it may help lower homocysteine levels.
  • Women with irregular periods or PCOS (polycystic ovary syndrome): Products that combine it with inositol are popular.
If you're planning a pregnancy, remember the timing. The neural tube starts forming before you even know you're pregnant. So start 1–3 months before pregnancy and keep taking it at least through week 12. Continuing after that is also helpful.

Folate daily dose: how much do you need?

Bottom line: 400 mcg a day is enough. The upper limit is 1,000 mcg.

SituationDaily dose
General health400 mcg
Planning a pregnancy, early pregnancy400–1,000 mcg
You take seizure medicine (anticonvulsants) or have had a baby with a neural tube defectYou need more — always ask your doctor about the dose

Here's a common mistake. Multivitamins, B complexes, and prenatal vitamins already contain folate. If you take extra folate on top, it's easy to go over 1,000 mcg without realizing it.

  1. Add up every 'Folate' or 'Folic Acid' number on the labels of what you take now.
  2. If the total is 400 mcg or more, you don't need to buy folate separately.
  3. If you've been taking a multivitamin with folate since early pregnancy, you don't need extra.

Methylfolate (MTHF) vs folic acid: which should you choose?

Regular folic acid has to go through several steps in your body to become the active form before it can be used. But quite a few people convert it slowly because of a variant in the gene that handles this conversion (MTHFR). That's why active forms that can be used right away were developed.

TypeWhat it's likeBest for
Folic acid (synthetic folate)The most common, also used in medicines. Inexpensive. Stay under the 1,000 mcg upper limit.Pregnancy planning, people who want value
Active folate (5-MTHF, L-methylfolate)Used right away with no conversion. Less worry about leftover folate building up.People concerned about slow conversion, people who need higher doses
Folinic acidAnother active form. Liquid versions make it easy to adjust the dose drop by drop.Children who can't swallow pills
Natural folate (from plants like lemon)'Natural,' but absorbs poorly, so you may need more.Little reason to insist on it

Here's a simple take on where opinions differ.

  • You'll often hear that people with an MTHFR gene variant must take the active form. But the US CDC (Centers for Disease Control and Prevention) says that even with a variant, 400 mcg of regular folic acid is enough. It also says there's no evidence that unconverted leftover folic acid is toxic.
  • So regular folic acid is enough for pregnancy planning. Ads claiming 'active form is a must' are overstated.
  • That said, the active form has almost no side effects and is used right away, so if the price gap is small, choosing the active form is a good option.
If the label says 'Folate (as 5-MTHF)', 'L-Methylfolate', or 'Quatrefolic', it's the active form. If it only says 'Folic Acid', it's regular folic acid.

Folate overdose: side effects and cautions

Folate is a water-soluble vitamin, so it's relatively safe. But avoid taking more than 1,000 mcg of regular folic acid for a long time.

  • It can mask a vitamin B12 deficiency. With a lot of folate, warning signs like anemia may not show up even if you're low in B12. Meanwhile, nerve problems can progress.
  • Some experts also say long-term high doses may have negative effects on nerves or immune cells.
  • If you take seizure medicine (anticonvulsants), you need more folate, but don't increase the dose on your own; decide it with your doctor.
People who take a multivitamin + B complex + folate together are the most likely to go over the limit. Take out all your labels once and add them up.

Supplements that pair well with folate

  • B vitamins (B6, B12): They work as a team with folate to process homocysteine. If your goal is blood vessel health, taking them together as a B complex is efficient.
  • Inositol: For women with irregular periods or PCOS, there are many products that combine it with folate in one pill.
  • Multivitamin: Many already contain folate, so check the amount before taking more separately.

Medications and conditions to watch

  • Check Seizure medicine (anticonvulsants) — Increases how much folate you need — decide the dose with your doctor
  • Note Multivitamin — Combined with the folate in your multivitamin, it's easy to go over the upper limit (1,000 mcg) — add up the labels
  • Note B vitamins (B complex) — B complexes also contain folate, so they overlap — check the total

Combinations

Works well with: B vitamins (B complex), Inositol

Frequently asked questions

When should I start taking folate?

If you're planning a pregnancy, start 1–3 months before. The neural tube forms before you even know you're pregnant. It's best to take it every day at least through week 12.

What's the recommended daily amount of folate?

400 mcg a day is the baseline. If you're planning a pregnancy, take 400–1,000 mcg. With regular folic acid, it's best not to go over 1,000 mcg.

Do I really need active folate (methylfolate)?

Not necessarily. The US CDC's position is that even with a gene variant, 400 mcg of regular folic acid is enough. But the active form is used right away and has almost no side effects, so it's worth choosing if the price is similar.

If I take a multivitamin, do I still need folate?

If your multivitamin has 400 mcg or more of folate, you don't need extra. Check the Folate number on the label first.

Are there side effects from taking too much folate?

Taking more than 1,000 mcg of regular folic acid for a long time can mask a vitamin B12 deficiency. Check the total so it doesn't overlap with your multivitamin or B complex.

Can men take folate?

Yes. Folate works with B6 and B12 to lower homocysteine, so it may also help people looking after blood vessel health. Usually a B complex or multivitamin covers it.
